At the time of this performance improvement project, Dr. Bradley was a pharmacy practice resident and Ms. Tuttle was a clinical pharmacy specialist, both at the Erie VA Medical Center in Erie, PA. Currently, Dr. Bradley is a clinical pharmacy specialist at the Erie VA Medical Center and Ms. Tuttle is a pharmacy program specialist in clinical informatics and pharmacy benefits management at the VA Central Office in Washington, DC. Dr. Chrymko is a clinical pharmacy specialist at the Erie VA Medical Center and a fellow of the American Society of Health-System Pharmacists.
