Surgery improves survival in localized prostate cancer

Key clinical point: Radical prostatectomy is associated with a small but significant reduction in all-cause mortality compared with observation in patients with localized prostate cancer.

Major finding: Radical prostatectomy significantly lowered the risk for all-cause mortality at 18-year follow up compared with observation (hazard ratio [HR], 0.84 [P = .044]; absolute risk reduction, 5.7 percentage points at 22.1 years).

Study details: 18-year follow-up data of the randomized PIVOT study which included 731 patients with localized prostate cancer managed with either radical prostatectomy or observation.
