Surgery improves survival in localized prostate cancer

Key clinical point: Radical prostatectomy is associated with a small but significant reduction in all-cause mortality compared with observation in patients with localized prostate cancer.

Major finding: Radical prostatectomy significantly lowered the risk for all-cause mortality at 18-year follow up compared with observation (hazard ratio [HR], 0.84 [P = .044]; absolute risk reduction, 5.7 percentage points at 22.1 years).

Study details: 18-year follow-up data of the randomized PIVOT study which included 731 patients with localized prostate cancer managed with either radical prostatectomy or observation.

Disclosures: This study was supported by Department of Veterans Affairs Cooperative Studies Program, National Cancer Institute, and the Agency for Health Care Research and Quality. Dr. Wilt is an employee of the VA.
