Chronic Progressive Swelling of Legs and Feet

A man presents with fever and severe, painful, erythematous, blistering rash of 2 days' duration that exhibits a positive Nikolsky sign; a 68-year-old man with a history of obesity and congestive heart failure is troubled by chronic progressive swelling of his legs and feet.
