Need an edge with T2DM? The case for team-based care

The Community Preventive Services Task Force is recommending team-based care for patients with type 2 diabetes, citing evidence linking this approach to lower HbA1c levels, as well as lower blood pressure and cholesterol levels. This audiocast details the evidence supporting this recommendation and describes another intervention that the CPSTF says can also benefit these patients.
